The referral process can be initiated by health care professionals, family members, friends, clergy, or by patients themselves. After a referral is made, we contact the patient’s primary care physician to get the clinical information needed to determine a patient’s diagnosis and appropriateness for hospice care. Once we receive the physician’s order, our nursing staff contacts the patient and family to set up an initial visit and complete the admission.
